2. For subscribers, provide your member ID#, birth date, zip code, and last 4 digits of your social security number. For dependents, provide your member ID#, zip code, and the month and day of the subscriber's birthday.
3. Once you have completed the registration process, you will be able to log in and access your personal benefit information.
