1. Click on a Health Tracker image on the home page or Take Charge of My Health page, or on the link under the Take Charge of My Health tab.
2. Choose the Health Tracker you would like to use and follow the instructions.
3. Once you have entered your information in the Health Tracker chart you can Print to review them with your provider.
