HEALTH PLAN TO BEGIN USING UNIQUE IDENTIFICATION (UID) NUMBERS ON APRIL 1, 2008
Effective April 1, 2008, the Painters & Allied Trades D.C. #35 Health Plan will begin using Unique Identification (UID) numbers rather than your redacted social security number. All medical claims submitted to Carelink PPO April 1, 2008 or later MUST have this new UID number or the medical claim(s) will be rejected. It's very important that you provide this new UID number to your doctor, hospital or any medical provider you visit after April 1, 2008.
