INFERTILITY TREATMENT
EFFECTIVE January 1, 2011

Infertility treatment including prescription medications will be paid at 50% to a lifetime maximum of $10,000. Diagnostic testing to confirm an infertility diagnosis will not be included in the lifetime maximum.
