Exclusive Provider Organization (EPO) Plan

Our Exclusive Provider Organization offers members quality health care coverage on a self-insured basis that meets some employer's needs.

Members choose a primary care physician who helps coordinate their care through our full network of more than 18,000 doctors and more than 80 hospitals.

These plans typically include comprehensive coverage for care, including:

• Doctor's office visits and specialist care
• Well Child care
• OB/GYN care
• Immunizations
• Lab tests
• Pharmacy coverage

Members also have access to our wealth of additional programs and benefits, including:

• Wellness discounts on fitness memberships, Weight Watchers discounts, seminars and boys and girls club memberships
• Savings on massage and acupuncture
• Health programs, including disease management for serious and chronic conditions.

With the EPO plan design, we also offer several levels of coverage that vary from Premium to Value to Basic, depending on the level of inpatient and outpatient copayments.
