If you've been diagnosed with diabetes, the Tufts Health Plan Taking Charge™ Diabetes Program can help you better understand and control your condition. The program works to:

• Increase your self-management skills to achieve optimal control of your diabetes
• Raise your awareness of necessary exams to screen for potential diabetes-related conditions
• Provide educational materials on diabetes-related health topics
• Support the patient-doctor partnership in the management of your diabetes

Program Details
The Taking Charge Diabetes Program is available to Tufts Health Plan self-insured Members and to Tufts Health Plan Medicare Preferred Members.

Educational mailings and case management services for those at high risk are part of the program.

Eligible members also receive an educational mailing that includes preventive diabetic care, self-management tools, information on diabetes and depression and a depression self-assessment screening tool.

Providers receive information regarding the screening history of their identified members and information on screening recommendations.
